COVID-19 Vaccine - Pre-appointment information
Before your COVID-19 vaccination please complete a consent form and a new patient registration form if you are new to the practice.
Step One: Download the Hotdoc app and create an account for the patient who is receiving the vaccine. Your details must match your Medicare card.

Before & After your COVID-19 Vaccination
Please make sure that you have completed your consent form and new patient form (if new to the medical centre). The details on these forms must match those on your medicare card.
Arrive 10 minutes before your vaccine appointment time.
Bring proof of your eligibility & your Medicare card.
Wait 15 minutes following your vaccination.
Children's 5 - 11 years Vaccine Information
Please bring your medicare card, masks for adults, hat, water and umbrella (if the weather permits)
For COVID safe purposes, please ensure that only one guardian accompanies your child to their appointment.
Hotdoc will send you a pre-appointment email where you will need to complete a consent form for the child and a new patient registration form (if new).
To find out if you are eligible to receive the vaccine now, visit https://covid-vaccine.healthdirect.gov.au/eligibility and complete the COVID-19 Vaccine Eligibility Checker.
You can then make a booking to receive your vaccine.
COVID-19 vaccinations are voluntary and free.
When it is your turn to get vaccinated, you can book easily online through the Hotdoc app to get your first and second doses of the vaccine.
If you are unable to book online you can call us on (07) 3273 5022 to book your appointments.
If you have traveled overseas in the last 14 days, or have been with someone who has, and you have a fever (equal or greater than 38 degrees), cough, sore throat or difficulty breathing, please ring ahead of time to book your appointment letting the staff know of your travel history and symptoms.

Sunnybank Hills Medical Centre has created an outdoor waiting area for our patients. Simply notify reception that you will be waiting outside and your family doctor will collect you for your appointment. The health and safety of our patients remains our highest priority.
Temporary Medicare Benefits Schedule (MBS) and Department of Veterans’ Affairs (DVA) items will allow doctors, nurses and mental health professionals to deliver services via telehealth.
The new MBS items will allow all medicare eligible patients to access essential health services in their home which will reduce the risk of exposure to COVID-19 in the community.
Sunnybank Hills Medical Centre is a mixed billing practice. Patients under the age of 16 or concession card holders will be bulk billed for telehealth appointments before 5pm Monday – Friday.
All other telehealth appointments may be privately billed. Please contact our friendly receptionists for more information.
